How do I submit my record?
Each year, you need to complete a record card with details of the learning activities you have undertaken during the year. This includes the compulsory 35 hours of activities, with supporting evidence.
When you’ve completed your record card, sign and print your name and return it with the required evidence no later than 31 July 2015 to:

Alternatively, you can send your record card and evidence by e-mail. All evidence should be scanned where necessary and attached with your record card, and then e-mailed to firstname.lastname@example.org
